The health watchdog has said Savita Halappanavar may still be alive if a series of opportunities to detect her deteriorating condition hadn't been missed.
The HIQA report identifies 13 opportunities that staff at University Hospital Galway could have detected her problems.
It concludes that there were failures in the provision of the most basic care to Mrs Halappanavar in the days before her death.
